Kek, L4D got a small patch yesterday (well, overnight)...
GET READY FOR THE WHINE, IN KEK 4 DEAD!
March 25, 2009 - Left 4 Dead Update ReleasedUpdates to Left 4 Dead have been released. The updates will be applied automatically when your Steam client is restarted. The specific changes include:
Game and client changes
Introduced fatigue to the melee attack in versus mode. You will see a HUD display when your melee swing is cooling down
Boomer vomit will now pass through common infected
Addressed some surround sound detection issues
Fixed a rare crash when exiting Left4Dead on Windows Vista
Fixed a case where the complete all expert campaigns achievement was not being rewarded
Dedicated server changes
Corrected an issue where Windows based dedicated were sending spurious shutdown notifications to the master causing them to delist temporarily
Fixed dedicated servers not resetting the versus team swap correctly--you could join a dedicated server that had a previous game that was swapped and arrive on the other team than what was selected in the lobby
Renamed the tags for sv_search_key to use "key:" in the server tags instead of "sv_search_key." This should help with issues of the 63 character limit of server tags
Fixed an occasional server crash related to voting
We have been going on about melee having some kinda fatigue or stamina bar since day 1, given a boomer swarm can potentially do zero damage to a huddled group in a corner, that hunters also cannot penetrate.
Also, if my analysis is correct, during some of the set piece sections (flick a switch, wait for an elevator etc) the adoption of tactics involving huddling in a corner and meleeing became very common (cause it worked). This may now cease.
Well, im gonna try out this fatigue bar now, but i imagine you either 1) simply cannot huddle in a corner anymore and will have to do them as Valve intended (molotovs to block sections, people on the minigun) or 2) have some epic co-ordination of the survivors so only 1 or 2 people melee at once, then when they are on cooldown, the other 2 step in, rinse and repeat.
We shall see. Will report back in a bit on this melee bar.

I'll have to restart my server for it to update and try the new "key:" system. Melee fatigue *should* make it more strategic.. As you say you need coordination - You don't want the entire team meleeing at once.. just 2 doing it, then when they get "tired" the other two take over.. Molotovs will also help. I think when swarmed just pistol shoot them (unlimited ammo), then melee WHILST reloading!

The free Survival Pack DLC arrives on Xbox 360 tomorrow "at roughly 2am PST," bringing the fast-paced Survival mode to zombie-killers alongside the long-awaited addition of the Dead Air and and Death Toll campaigns to Versus.
PC owners will have to wait, but, assuming all goes to plan, not for long. According to Valve, the PC debut of the free add-on is "targeted for release later in the day."
Rumours are that the PC version wil release roughly 7-8pm GMT
